HEALTH/MEDICAL:
MSL follows the guidelines of Jefferson County Public Schools for requirements related to immunizations, physical exams and eye exams. Please refer to the JCPS  web site for information regarding immunizations, exams, etc.

REQUIRED FORMS FOR MSL STUDENTS:
      Birth certificate – issued by state of birth, not hospital,

      Kentucky Physical Exam –through 4th grade
      Kentucky Physical Exam – 5th grade and older
